Emergencies/After Hour Policies
If you have a life threatening emergency, call 911 immediately. For all other urgent care and/or after hours needs, please call 305-669-4426 and our on call physician will be glad to assist you.
Appointment Information

Arriving Late
If you are more than 15 minutes late for an appointment, your doctor will determine if time still allows for you to be seen.
Appointment Cancellation
If you are unable to keep your appointment, please contact our office 24 business hours prior to your schedule appointment date, so that this time may be made available to another patient.
Referrals
Before your initial visit, please check with your insurance carrier to see if you need a referral form from your primary care physician. We do require an active referral to be on file prior to your visit. Referrals can be faxed to

Orthopaedics and Rehabilitation Excellence of Miami participates with most major insurance companies. Your particular insurance may require a specific referral made by your primary care physician. We require an active referral to be on file prior to your visit. Co-payments are expected at the time of service.
